Waiver and Release
I enjoy taking pictures and love to share. Below is a permission slip for me to be able to post picture of your adorable children on various sites.
I/We hereby consent to the use of photographs of ___________________________ (name of child) ___________________________ (name of child) to be posted on the provider's Facebook page.
I/We also hereby agree to waive and forego any right or entitlement of claim I/we might have to any compensation, fees, or other benefits except for a waiver fee of $1.00.
Further, by signing this waiver and release I/we certify that I/we am/are the legal parent or guardian of the child identified above.
